OCOM 7008 - Longitudinal Integrated Clerkship 5


The fifth in a series of longitudinal courses that guides students towards competence in patient-centered primary care. This course helps learners develop the skills needed to lead in health care delivery science, population health, communications, health care team leadership, health information technology and analytics, as well as, patient-centered medical home practice.

This course contains a component called Care Coordination and Navigation. Students will engage with Care Coordinators, nurse managers and other interprofessionals. Using population health skills from health systems science training, students will engage patients across the care continuum to work towards disease management, outcome improvement and return to wellness.

Course Outcomes

  • Students will be able to describe the benefits and clinical application of telemedicine.
  • Students will be able to identify the communicaiton needs of consultants and other parts of the health care system.
  • Students will be able to assist patients in connecting with peer support groups or other appropriate community resources.
  • Students will be able to assist patients with health behavior change and education about chronic disease management.
  • Students will be able to develop caring relationships with a panel of patients.
  • Students will be able to develop evidence-based personal care plan for patients.
  • Students will be able to create patient care plans through collaboration with interdisciplinary care teams.
