During this workshop, we will discuss and plan care coordination for those most in need through actual patient case studies. This workshop is co-facilitated by Dr. Fran Blais and Isi Ikharebha, Executive Director at the Physicians CareConnection with guest facilitators on each campus. 

This event will be held on November 17 from 5:00-7:30 p.m. facilitated live on each campus.

Food will be served from 5-5:30 and the workshop will take place from 5:30-7:30. 

The purpose of this workshop is to challenge you to identify the social determinants of health that are barriers to providing effective and successful medical care to patients. Your task will be to identify community resources that are available in your community to address these barriers and how to guide the patient to access these resources for assistance in their care. Through several case discussions, on each campus, care coordinators will assist you in identifying the resources in your community critical to the management of underserved patients.
