OCOM 8007 - Longitudinal Integrated Clerkship 7
The seventh in a series of longitudinal courses that guides students towards competence in patient-centered primary care. This course helps learners develop the skills needed to lead in health care delivery science, population health, communications, health care team leadership, health information technology and analytics, patient-centered medical home practice, as well as in the community and in community agency management.

Course Outcomes
- Student will be able to explain the benefits of interdisciplinary health care teams.
- Student will be able to identify the communicative needs of consultants and other parts of the health care system.
- Student will be able to assist patients in connecting with peer support groups or other appropriate community resources.
- Student will be able to assist patients with health behavior change.
- Student will be able to apply team-based principles to patient care.
- Student will be able to support and visit a panel of patients in a longitudinal fashion.
- Student will be able to engage with and support a community agency in providing care for a particular population of patients towards improved outcomes.
- Student will be able to utilize literature in addressing disease and patient-oriented evidence in care of patients.
- Student will be able to present at least on patient-centered, disease specific presentation to peers and others in an interactive format.
- Student will be able to identify and apply population health data to address health care disparities.
- Student will be able to integrate the critical appraisal of the medical literature with clinical expertise for the patient’s unique biology, values, ethnicity and circumstances.
- Student will be able to perform regular self-assessment and select educational activities best designed to address identified deficits in competency and performance, in alignment with standards set by the profession
- Student will be able to demonstrate the use of tools employed in quality improvement.
- Student will be able to utilize these tools to complete both a community population health project and complete at least one quality improvement cycle in this project.
