Overview

HCOM students must complete a Health and Technical Standards Attestation Form prior to matriculating to the Heritage College. Additional H&T forms may be submitted throughout the student's medical education as needed. The forms are submitted through the HCOM Student Portal and reviewed by the Office of Medical Education. Any necessary accommodations must be completed through the appropriate office (Accessibility Services or Equity and Civil Rights Compliance Equity and Civil Rights Compliance). 

Accessing the Student Portal and Logging In

The Student Portal can be accessed using the following link:  https://ouhcom.my.site.com/students/ . You will use your OHIO email and password to log in. 

Upon logging in, you'll land on a homepage similar to the one pictured below. From here select "Health & Technical Standards".

Submitting a Health and Technical Standards Form

The page that loads will provide detailed instructions and information regarding the Health and Technical Standards Attestations, please be sure to read it thoroughly. To submit your first or a new form, click the " Create New Health and Technical Standards Form" button. 

On the following page, answer the questions to the best of your ability.

If you select "Yes, with accommodations" for any of the sections, you will need to provide additional information that you believe will assist the college in understanding your answer, and you will need to reach out to the appropriate office to help you obtain the appropriate accommodation. 

Lastly, you will be required to submit an electronic signature before you are able to submit the form. 

Review of the Form

Once the form is submitted, it will go 'read only', meaning that no additional changes can be made and the status will be reflected as "Submitted". If "No" was selected for any of the sections, the Office of Admissions may reach out to you for further clarification.

 The Office of Medical Education (OME) will review the form and update the status to "Received by OME". If they have any questions or concerns about your submission they will be sure to reach out to you. 

Should you need to submit a new form at any time, you can repeat these steps.
