This worksho p is co-facilitated by Dr. Fran Blais and Isi Ikharebha\, Executive Directo r at the Physicians CareConnection with guest facilitators on each campus. \n\nThis event will be held on November 17 from 5:00-7:30 p.m. facilitated live on each campus.\n\nFood will be served from 5-5:30 and the workshop wi ll take place from 5:30-7:30. \n\nThe purpose of this workshop is to challe nge you to identify the social determinants of health that are barriers to providing effective and successful medical care to patients. Your task will be to identify community resources that are available in your community to address these barriers and how to guide the patient to access these resour ces for assistance in their care.
