OCOM 7008 - Longitudinal Integrated Clerkship 5
The fifth in a series of longitudinal courses that guides students towards competence in patient-centered primary care. This course helps learners develop the skills needed to lead in health care delivery science, population health, communications, health care team leadership, health information technology and analytics, as well as, patient-centered medical home practice. This course contains a component called Transitions of Care (ToC) which gives learners exposure to hands-on experiences in a variety of care systems that help support patient management in a longitudinal fashion.
Course Outcomes
- Describe the role of other disciplines in the discharge planning process and work with the interdisciplinary team to coordinate the patient’s discharge.
- Develop caring relationships with a panel of patients.
- Identify patient safety issues surrounding discharge planning (including medication reconciliation, functional assessments and how to interpret these assessments) to predict patients at risk for poor outcomes.
- Identify the communicative needs of consultants and other parts of the health care system.
- Apply methods of communication appropriate for remote patients.
- Assess the self-management needs of patients with chronic illness.
