OCOM 7009 - Longitudinal Integrated Clerkship 6


The sixth in a series of longitudinal courses that guides students towards competence in patient-centered primary care. This course helps learners develop the skills needed to lead in health care delivery science, population health, communications, health care team leadership, health information technology and analytics, as well as, patient-centered medical home practice.

Course Outcomes
  • Develop caring relationships with a panel of patients.
  • Identify the communicative needs of consultants and other parts of the health care system.
  • Use an evidence-based approach for chronic disease management and preventive health care.
  • Use evidence-based decision support tools at the point of care during patient visits.
  • Use IT tools to analyze patient and practice data to improve health outcomes.
  • Assess the self-management needs of patients with chronic illness.
  • Provide appropriate disclosure to patients when errors occur.
