OCOM 6007 - Longitudinal Integrated Clerkship 1


The first in a series of longitudinal courses that guides students towards competence in patient-centered primary care. This course helps learners develop the skills needed to lead in health care delivery science, population health, communications, health care team leadership, health information technology and analytics, as well as, patient-centered medical home practice. This course contains a component called Transitions of Care (ToC) which gives learners exposure to hands-on experiences in a variety of care systems that help support patient management in a longitudinal fashion.

Course Outcomes
  • Define the basic concepts of practice management.
  • Describe the team approach to patient care.
  • Explain the basic concepts of the patient-centered medical home.
  • Explain the roles of trans-disciplinary team members as they apply to the scope of practice.
  • State the benefits of a personal clinician to the health of individual patient and patient populations.
  • Identify available resources and help patients select appropriate community resources and peer support groups.
  • Support and visit a panel of patients in a longitudinal fashion.
  • Assist patients in connecting with peer support groups or other appropriate community resources.
  • Summarize patient coaching methods and apply these methods to the assigned cohort of patients in a supervised clinical setting.
