How to build an accessible survey
Plan for alternative access
A survey's content or delivery mode may require alternative access for a participant. You should plan ahead to have accessibility accommodations available. Examples include:

Creating a copy of the survey in Microsoft Word or Plain Text format that can be used with assistive technologies like screen readers.

Offering alternate methods of participation like email or phone call.

Having staff available to provide accommodations like recording responses or reading questions and responses aloud.
Your alternative access accommodations should be described in the introductory text of the survey and/or in the survey distribution communications.
Use accessible question types
Qualtrics has acknowledged that some of their question types are not accessible. In most cases, inaccessible questions can be replicated with an alternative version that is accessible
