Update My Personal Information

Name

Students are required to provide their legal names on their applications for admission. After enrollment, students may process official name changes or corrections with the Office of the University Registrar. A change of legal name requires an official document or court order verifying the correct information at the time the request is made. Name changes are processed by the Office of the University Registrar only. One of the following documents (copies) may be submitted for verification of the legal name change to the Office of the University Registrar, Chubb Hall.

  • Passport (required for international students in non-immigrant status)
  • Court action document
  • Divorce decree (the appropriate page of the decree)
  • Marriage certificate
  • Valid driver's license and social security card (you must submit both) or
  • Federal or state issued identification card and a social security card (you must submit both)

A valid passport is required for a change of name for an international student. International students (F-1 or J-1 status) must notify Office of International Student and Scholar Services (ISSS) of their change of name.

Preferred Name and Pronouns

Students can update their preferred names and pronouns in MyOHIO Student Center . After logging in, select on the "Names" link in the Personal Information section.

Faculty and Staff may update their preferred first names in My Personal Information . After logging in, expand the "My Personal Information" folder, select "Personal Information" and then in the "Basic Details" section, select the Update button.

Addresses

It is the student's responsibility to maintain a current address on file with Ohio University. Student can easily update their address by accessing MyOHIO Student Center and navigating to the Personal Information section.

If a student has trouble accessing the MyOHIO Student Center or updating their address, address updates may be reported to Registrar Services by completing a Change of Address [PDF] form. The completed form (must include student signature) can be mailed or faxed to the Office of the University Registrar, Chubb Hall, Ohio University, Athens, Ohio 45701; fax 740.593.0216.

You may also update your Current Living Address on your desktop or mobile/tablet device. 

It is very important to keep your home and mailing addresses and telephone numbers (including cell phone) current in order for the University to communicate important information to you. Please note that, as a student, if you provide the University with a cell phone number (public or private) and you are an Athens campus student, you will be subscribed automatically to the emergency communication system. If you are a regional campus student, please check with your regional campus student services office for available signup for emergency communications. 

Note:International students in F-1 or J-1 status are requiredto use MyOHIO Student Center to update their "SEVIS U.S." and "SEVIS Foreign" addresses with Ohio University, within ten days of moving, to meet immigration reporting requirements. Legal Sex

Students provide their legal sex on their applications for admission. After enrollment, students may process sex changes or corrections with the Office of the University Registrar. A change of sex requires an official document verifying the correct information at the time the request is made. Sex changes are processed by the Office of the University Registrar only. One of the following documents (copies) may be submitted for verification of the sex change to the Office of the University Registrar, Chubb Hall.

  • Birth Certificate
  • Driver’s License
  • Federal/State Photo ID
  • Medical documentation
  • Passport

Note: Change of name is a separate process, not necessarily related to sex change. The appropriate process should be used for each change. Change of name from one commonly associated with one sex to one commonly associated with the other is not sufficient reason to change the sex in the records.

Personal Information

All changes of student personal data must be reported to the appropriate office. Please complete the Personal Data Validation Form . Forms are available at Registrar Services, first floor, Chubb Hall or regional campus student services offices.

Changes or corrections of name, Social Security number, or birth date require official documents and/or court orders verifying the correct information at the time the requests are made. These changes or corrections (name, Social Security number, or birth date) are processed only by the Office of the University Registrar, Chubb Hall.
