Service Maintenance Permit Request Form
Departments wishing to request a service maintenance permit for use in a University-owned state vehicle must submit a request to be considered for parking options within the Service Maintenance spaces on campus. All fields listed below must be completed to be considered. Incomplete forms cannot be accepted.
Please be advised: submission of a request for SM permits does not guarantee issuance of requested permits. SM permits are issued at the discretion of Transportation & Parking Services.
