This form must be submitted to Debra Fanning (fanning@ohio.edu) at least 2 weeks prior to the request date for your PROPOSAL DEFENSE. The faculty chair of the thesis/dissertation committee must sign the form prior to submission.

Final Thesis and Dissertation defenses have priority for scheduling in 126 over Proposal Defenses. Therefore, students making arrangements for the proposal meeting should be aware that, should a Final Defense be requested at the same time, a different room will need to be scheduled.
