Address Update

Students on an F or J visa needing to update address and tax information should refer to the information on the F or J Visa Address Update page .

All other employees needing to update or correct address information in My Personal Information (MPI) , should follow the directions below: 

Residents of Ohio, Kentucky, and West Virginia

1. Please download and complete the appropriate form from the links below, based on the state of your  new address.
   1. Ohio Residents
   2. Kentucky Residents
   3. West Virginia Residents
2. Upload the completed documents to University Human Resources via the secure file upload site . 

Residents of all other states

Please contact the University Human Resources Service Center at uhr@ohio.edu or 740.593.1636 to request the appropriate forms for your move. 

Employees needing to update the Inside City Limits Flag ONLY

If your information is correct in MPI but you need to update the "Inside City Limits flag," please download and complete the City Tax Form and upload the completed documents to University Human Resources via the secure file upload site .  

Please note: Submitted changes will be reflected in MPI by the pay date following completion and upload of documentation.
