Community Diabetes Programs
The Diabetes Institute provides diabetes education and prevention programs to individuals with or at risk for diabetes with a focus on improving access to these services. These programs provide ongoing support and equip people with tools they need to prevent or manage diabetes.
-
Diabetes Navigation
Our diabetes nurse navigator works with youth and adult clients who have type 1 or type 2 diabetes to help overcome health system barriers by offering individualized, patient-centered care coordination, social support, and assistance to clients and families.
-
Peer Support
Special events are held to help build peer support and connection among youth reached by the diabetes navigator and our diabetes camp. These in-person opportunities help to reinforce these connections throughout the year.
Our team includes a Certified Diabetes Care and Education Specialist and Registered Nurses who possesses comprehensive knowledge and experience in diabetes management, prediabetes, and diabetes prevention.
